The Social Work Case Manager is responsible for providing case management services to participants intended to promote self-sufficiency according to the participants’ identified needs and the programs’ focus. The Social Work Case Manager supports participants through a human centered and trauma informed approach, in addition they are engaging, culturally responsive and aware of the diverse needs of various cultures, races, ethnicities and populations.
The Social Work Case Manager will be an integral part of our rapid rehousing transitional housing program, providing intensive case management and support to participants. Utilizing a housing first approach, this role focuses on assisting participants in finding suitable housing in their preferred community. The Case Manager will create a safe, supportive, and welcoming environment to facilitate participants' transition to independence and self-sufficiency. Additionally, the Case Manager will help participants identify and secure appropriate housing in their chosen community, maintain knowledge of local housing resources and options, and collaborate with the Housing Operations Manager to maintain smooth program operations.

PROGRAM SUMMARY:
The Joint TH-RRH program aims to connect youth (ages 18-24) to safe, permanent, and inclusive housing. We focus on creating permanent connections, employment, education, and entrepreneurialism and promoting social and emotional well-being. Participants in RRH will receive 12 months of subsidized rental assistance in their chosen location, encouraging youth choice. During this period, the case manager will offer intensive case management and services to support their journey toward independence.
