The Social Work Case Manager is responsible for providing case management services to participants intended to promote self-sufficiency according to the participants’ identified needs and the programs’ focus. The Social Work Case Manager supports participants through a human centered and trauma informed approach, in addition they are engaging, culturally responsive and aware of the diverse needs of various cultures, races, ethnicities and populations.

POSITION & PROGRAM SUMMARY:
The Housing Our Youth (HOY) program provides comprehensive services to youth experiencing homelessness, with a focus on helping them achieve long-term stability and independence. Through a combination of temporary emergency shelter, long-term supportive housing, case management, and access to healthcare, education, and employment resources, HOY creates individualized pathways to housing and self-sufficiency.
The Social Work Case Manager provides case management services that promote self-sufficiency based on participant needs and program goals. Within the Housing Our Youth (HOY) program, they serve as both housing navigator and trusted support, guiding transition-age youth through securing stable housing by assisting with applications, advocating with landlords, connecting to flexible funding, and addressing barriers such as credit or documentation. Case Managers utilize the Homeless Management Information System (HMIS) and the 211 Community Information Exchange (CIE) to coordinate referrals, track progress, and ensure participants are connected to appropriate housing and supportive services. Using a trauma-informed and culturally responsive approach, they support participants beyond housing, linking them to employment, education, childcare, and health resources while fostering long-term stability and growth.
